Fast Track Transition

This program is funded by the State of Illinois’ Division of Rehabilitation Services and is focused on working with youth from 14 through 21 on skills necessary to assist in the transition from High School into college or career.

It’s called “Fast-Track” for a reason, qualification rests on some simple questions

  • Are you 14 through 21?
  • Are you in an educational program (High School, GED Program, College, Etc.)?
  • Do you have an IEP or 504 Plan?
  • Are you a current customer of the Division of Rehabilitation?

If the answer to these question are; YES, YES, YES, & NO, then you’re in.
